New Patient Registration

We are amending the patient registration process to reduce the spread of Coronavirus by limiting the volume of traffic in the Practice and reducing the amount of paper handled.

A process has been created to allow patients to complete the registration form online and then email it to the Practice.

Click the link below to download a registration form.  To help provide us with information about you could you also complete the New Patient Health Record Questionnaire.

If you wish you can pop into the surgery the receptionist can also provide you with a registration forms to complete and hand back into the surgery (posted back preferrably).

diaryIf you live in Troon, Loans, Dundonald, Monkton or Symington you may register with this practice. 

We would ask that you provide some form of identification when you register ie passport, birth certificate, photographic driving license or national identity card.

Before registering with a new surgery it is helpful to ensure you have a few weeks worth of medication from your previous practice.
